Program Title:
Annual Cancer Screening

WI CCC Plan Related Chapter / Priority:
Chapter 2: Screening and Detection
Priority A: Increase colorectal screening rates for those 50 years and older, especially in high-risk populations.
Chapter 2: Screening and Detection
Priority D: Increase prostate cancer screening rates for men 50 years and older, especially in high-risk

Program Description:
Work in partnership with Affinity Health System to provide annual cancer screening targeted at uninsured and underinsured. The screenings (all are free) include: PSA and DRE for prostate cancer, oral screenings, colorectal education and guiaic cards, and skin cancer screening.

Program Title:
HAT (Health Assessment Tool)

Chapter 1: Prevention
Priority B: Promote healthy lifestyles through nutrition and physical activity.

Program Description:

System wide employees annually fill out questionnaire relating to their current health practices and family history. They also have labs drawn, height, percentage body fat, weight, BMI,and blood pressure. Results are reviewed with health coach (plans for improvement if necessary). Employees who complete the HAT receive a discount on their health insurance premiums. Spouses of employees are also eligible to participate in the HAT.

Program Title:
Palliative Care

WI CCC Plan Related Chapter / Priority:

Chapter 5: Palliative Care / End of Life Issues
Priority A: Increase access to palliative care.

Program Description:
Palliative Care team is available for inpatients and outpatients at Appleton Medical Center and the Martha Siekman Cancer Center located there. The team consists of physicians, a Nurse Practitioner, 3 Social Workers, and Chaplain services.
