What is the Survivorship Forum?
The Survivorship Forum will be a half-day event hosted within the 2010 WI CCC Summit.  The Summit's afternoon plenary speaker, Dr. Karen Basen-Engquist, will double as the introduction to the Forum.  The large group presentation will be followed by a panel discussion with just the Survivorship Forum attendees.  The Survivorship Forum will focus on best practices in cancer survivorship.  The panel discussion will be guided by the priorities identified in the Survivorship Chapter of the WI Comprehensive Cancer Control Plan 2010-2015; raise awareness of survivors’ needs, educate and empower survivors, increase access to quality care and services, increase survivorship related data through surveillance and research.  The mission of the forum is to create a meaningful and outcome oriented program where cancer survivors and providers have an equal voice to listen and dialogue with each other. 

Who Should Attend the Survivorship Forum?
The target audience includes practitioners, survivors and caregivers, but we would encourage anyone interested in survivorship issues to attend.


REGISTRATION

There is no registration fee for the Summit, Survivorship Forum or Pre-Summit Reception. Continental breakfast, lunch and an afternoon snack will be provided for the Summit.  An afternoon snack will be provided for the Survivorship Forum.  Complementary appetizers and non-alcoholic beverages will be served at the Pre-Summit Reception - beer and wine will also be available for purchase.
