Program Title:
Cancer Screening Care Management Initiative

WI CCC Plan Related Chapter / Priority:

Chapter 2: Screening and Detection
Priority A: Increase colorectal screening rates for those 50 years and older, especially in high-risk populations.
Priority B: Increase rates of breast cancer screening, especially in high-risk populations.
Priority C: Increase rates of cervical cancer screening, especially in high-risk populations.

Chapter 3: Treatment
Priority B: Encourage adherence by physicians to cancer treatment guidelines.

Program Description:
To increase our breast, cervical and colorectal cancer screening rates for our patients, multiple improvement strategies are being piloted, including outreach reminder letters and calls, mailing out iFOBT kits, and point of service reminders. In addition, staff and physicians are asked to document preventive care in a designated part of our EMR.

Tobacco Cessation Care Management Initiative

Chapter 1: Prevention
Priority A: Decrease tobacco use and exposure to tobacco smoke.

Program Description:

To develop and implement systems that consistently identify Aurora patients with tobacco dependence and offer standardized methods to intervene and support patients who are ready to make a quit attempt.
